1. Assumption of Responsibility
The parent(s) and/or legal guardian(s) agree to instruct the minor participant to inspect the facilities and equipment before participating in any Mountain City Center for the Arts (MCCA) class, rehearsal, performance, camp, workshop, outreach activity, private lesson, or event. If the participant believes any condition or equipment is unsafe, they should immediately notify an MCCA staff member and refrain from participating until the concern has been addressed.

I understand and agree that if I believe any condition to be unsafe, I will immediately notify MCCA staff, take reasonable precautions to avoid the unsafe condition, and discontinue participation until it is safe to continue.

2. Acknowledgement of Risk
I/We fully understand and acknowledge that:
(a) Participation in MCCA classes, rehearsals, performances, camps, workshops, private lessons, outreach programs, and events involves inherent risks that may result in bodily injury, illness, partial or total disability, paralysis, death, or damage to personal property.

(b) The social and economic losses or damages resulting from these risks may be significant.

(c) These risks may arise from my own actions or inactions, the actions or inactions of others, the condition of the facilities or equipment, or the negligence of the Releasees identified below.

(d) There may be additional risks that are not known or reasonably foreseeable at this time.

3. Assumption of Risk
I/We voluntarily assume all risks associated with participation and accept full responsibility for any injury, illness, disability, death, or property damage that may occur, whether caused in whole or in part by the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

4. Release and Waiver of Liability
I/We hereby release, waive, discharge, and covenant not to sue Mountain City Center for the Arts (MCCA), Beyond the Stage, Inc., their officers, directors, employees, instructors, volunteers, contractors, agents, representatives, affiliated organizations, property owners, event hosts, and any other individuals or entities associated with MCCA activities (collectively referred to as the "Releasees") from any and all liability, claims, demands, actions, causes of action, damages, losses, costs, or expenses arising out of or related to participation in any MCCA activity, including but not limited to classes, rehearsals, performances, camps, workshops, outreach programs, private lessons, competitions, travel, and special events.

This release includes claims arising from bodily injury, illness, disability, death, or property damage, whether caused in whole or in part by the negligence of the Releasees or otherwise, to the fullest extent permitted by applicable law.

5. Emergency Medical Authorization
In the event of an emergency, I authorize MCCA staff or representatives to obtain emergency medical treatment for myself or my child if I cannot be reached immediately. I understand that every reasonable effort will be made to contact the parent or legal guardian before treatment whenever possible. I accept full responsibility for any medical expenses incurred as a result of such treatment.

6. Health Certification
I certify that I (or my child) am physically able to participate in MCCA activities and have disclosed any known medical conditions, allergies, medications, or limitations that may affect safe participation.

7. Photo and Media Release
I grant Mountain City Center for the Arts (MCCA) permission to photograph, video record, audio record, or otherwise capture my or my child's likeness during classes, rehearsals, performances, camps, workshops, outreach programs, private lessons, and other MCCA activities.
I authorize MCCA to use these photographs, videos, recordings, and other digital media in print, digital, website, social media, promotional, educational, fundraising, advertising, and marketing materials without compensation or additional permission. I understand that these materials become the property of MCCA and may be used indefinitely unless otherwise prohibited by law.

8. Acknowledgement
I acknowledge that I have carefully read this Release of Liability, Assumption of Risk, Media Release, and Indemnity Agreement. I fully understand its contents and understand that by signing this agreement I am giving up certain legal rights, including the right to bring legal action against the Releasees under certain circumstances. I sign this agreement freely and voluntarily without any inducement, assurance, or guarantee, and I intend my signature to serve as a complete and unconditional release of liability to the fullest extent permitted by law.
I affirm that I am at least 18 years of age or that I am the parent or legal guardian of the minor participant and have the legal authority to sign this agreement on their behalf.

By signing below, I acknowledge that I have read, understood, and voluntarily agree to all terms and conditions of this Release of Liability, Assumption of Risk, Media Release, and Indemnity Agreement.
